New Patients (forms)
For your first visit, you should bring current MRI or x-ray films, if available. We can also take x-rays in our office. If you are being treated for a knee condition, please wear or bring a pair of shorts. If you are being treated for a shoulder condition, please wear or bring a sleeveless top.
If you are unable to keep your appointment, please notify the office at least 24 hours in advance. No-shows and cancellations less than 24 hours in advance will be charged a $35 fee.
